Step 1: Whisk together the mushroom soup, milk and Parmesan cheese and set aside.
Step 2: Peel the potatoes and slice them into 1/4 inch thick rounds.
Step 3: Chop your cooked ham into bite sized pieces.
Step 4: Place the potatoes and the ham in the crock and sprinkle with all the seasonings.
Step 5: Pour the mushroom soup mixture over the potatoes and ham and place cover on crock pot.
Step 6: Turn the temperature on high and cook until the mixture starts to simmer, about 45 minutes to an hour depending on your crock pot.
Step 7: Turn the temp down to medium and cook for another 3 to 4 hours or until the potatoes are fork tender but not falling apart. Again this will depend on you crock pot.
