Step 1: Cook pasta according to package directions, then drain. Set aside.
Step 2: In a small pot heat heavy cream and cream cheese over medium-low heat. Whisk together until cheese is completely melted.
Step 3: Stir in garlic salt, and onion powder.
Step 4: Add shredded Swiss cheese.
Step 5: Whisk together until cheese is completely melted, and sauce is smooth.
Step 6: In a 9X13 inch pan add noodles, chicken, ham and bacon.
Step 7: Pour sauce over top and lightly stir.
Step 8: In a small bowl or Ziploc bag, mix bread crumbs with melted butter. Sprinkle bread crumbs over the top.
Step 9: Bake at 375 degrees F, for 20 minutes, until heated through and top is lightly browned.
